Step 1: REGISTER
Click here to set up your MaxWell Clinic Patient Portal account. The portal will serve as a communication hub to view or update your records and easily and securely communicate with your MaxWell Clinic team.
During this process, you’ll be prompted to create a username and password for future use.
You will receive an email to confirm your email address.
Step 2: SCHEDULE A CALL WITH OUR NEW PATIENT COORDINATOR
Click here to schedule your telephone appointment with our New Patient Coordinator. They will explain all things related to MaxWell Care, answer any questions, and help you set up your very first appointment!
Step 3: START YOUR PAPERWORK
Log into your Patient Portal, click on “New Patient Intake Forms” on the right side of the screen and start filling out your forms. Our clinicians are thorough and would like to know as much as possible about you before your visit together. At this time you’ll also select either our 3 month program or our 12 month program.
